TDS provides a monthly discount on communication services which may include broadband internet, phone (landline or wireless), or bundles to eligible low-income households.

Low-Cost Internet Plan - PCs For People
PCs For People provides a Low-Cost Internet Plan for low-income individuals needing internet connectivity in their households. A modem with unlimited 4G LTE internet service is provided to connect those using this program to the internet.

Supportive Services - Posada
A Posada case manager provides referrals to homeless individuals for shelter, food and employment assistance, review options for low-income housing and identify community resources and programs to obtain self-sufficiency.

Eligibility Criteria: Posada’s programs assist homeless families with children, young adults, veterans and individuals in Pueblo County. Everyone must meet the definition of “homeless” to receive services. Must be a Pueblo County resident to receive services.​

Fort Lyon Supportive Residential Community Program - Colorado Coalition for the Homeless
The Fort Lyon Supportive Residential Community Program provides recovery oriented transitional housing to homeless individuals. The program combines housing with peer support, educational, vocational and employment services for up to 300 homeless and formerly homeless persons from across the state of Colorado, with an emphasis on serving homeless veterans.

Eligibility Criteria: Must be seeking drug recovery, be actively homeless, and willing to live in transitional housing. Special consideration is put on serving homeless veterans.
Application/Intake Process: If you are experiencing homelessness and would like to enroll in the Ft. Lyon program, please consult with your case manager, homeless service provider, or health care provider about a referral to the program.

Transitional Housing for Families - Partners in Housing
Provides a 1-year transitional housing and supportive services program for homeless families with minor children. Also assists homeless pregnant women and homeless families who are in the process of having their children placed back into their custody.

Energy Bill Payment Assistance - Energy Outreach Colorado

Electric & Gas Affordability Programs through Xcel
Non-emergency energy assistance programs are administered and supervised by Xcel Energy. Click this following link to be directly sent to Colorado's Low-income Energy Assistance Program (LEAP) website.

Eligibility Criteria: If your income is up to 60% of the state median income level, you may qualify for heating assistance through LEAP. The state median income level used for the 2023-24 LEAP season was released by the U.S. Department of Health and Human Services in July 2023. The amount of the energy assistance benefit varies depending on a variety of factors, including the primary heating fuel costs and income.
Application/Intake Process: The 2023-24 LEAP season is now open. Applications are accepted until April 30, 2024.
LEAP accepts applications between November and April. Click below to learn more about applying for LEAP benefits, including a checklist to help you prepare your application. Provides free legal services for different housing situations such as: Evictions, Landlord/tenant disputes, & Other housing issues.

Provides homebuying education and and loans to those who live in Pueblo County who are preparing to buy a home.

Additional Information: NeighborWorks also offers a Rehab Loan Program. The Rehabilitation Loan/Emergency and Accessibility Loan Programs assist low income homeowners in financing basic home improvements that directly affect the safety, habitability, energy efficiency or accessibility of their homes.

 

Food Assistance:

Commodity Supplemental Food Program (CSFP) - Housing And Human Services Pueblo County
The Commodity Supplemental Food Program (CSFP) provides nutritious USDA foods. CSFP food boxes are distributed monthly, and contain 9-10 staple food items such as cheese, rice, milk, canned fruits, canned vegetables and peanut butter.

Eligibility Criteria: Eligible applicants:
- Adults 60 years old and older
- You must have a combined gross income which does not exceed the maximum income for the applicable household size.  View household size and income requirements. 
- Re-certification is required every 6 months to confirm that you wish to continue to receive Everyday Eats benefits and that your financial situation has not changed. Your Everyday Eats site manager will assist you with re-certification.
- Everyday Eats recipients are automatically eligible to receive emergency food items through  The Emergency Food Assistance Program (TEFAP).

Wait Time: Give yourself an hour to complete the initial intake survey. If  you are found to be eligible for services, an agent will reach out to your directly either by phone or email to schedule an upcoming food appointment pick up time.

Provides a food pantry both at their location as well as a Mobile Pantry. Sign in sheets must be filled out completely (this is the only requirement, no income verification). The Emergency Food Assistance Program (TEFAP) helps supplement the diets of low-income Americans by providing emergency food assistance at no cost.

Weatherization Program - Housing and Human Services Pueblo County
Pueblo County's Weatherization Department works with the Colorado's Energy Office Weatherization (CEO) Program to improve the energy efficiency of residential structures. Energy costs weigh heavily on low-income households. The CEO program provides energy upgrades to income-qualified homes to help keep energy costs low. Types of Weatherization Services: Attic Insulation, Sidewall Insulation, Perimeter/Rim Joist/Floor Treatment, Health & Safety Inspections

Eligibility Criteria:
- Non-emergency medical transportation is only available for select Medicaid/Medicare recipients, who also have Title XIX coverage.
- ALL Colorado Medical Assistance Program clients are eligible for NEMT services, unless the client falls within one of these eligibility groups on the date of the trip: Qualified Medicaid Beneficiary (QMB)
- ONLY Special Low Income Medicare Beneficiary (SLMB)
- ONLY Medicare Qualifying Individual-1 (QI-1)
- ONLY Old Age Pension (OAP – State Only) Child Health Plan Plus recipients are NOT eligible for NEMT services.
PACE clients receive transportation through their PACE organization, and are NOT eligible for NEMT services.
Application/Intake Process: In order to book a ride with Peak Family Transport, the patient will need the following: A state-issued photo ID A Health First Colorado member ID number A valid social security number An in-person appointment with a medical facility, of which they have no other means of travelling there. State coverage plan through Health First Colorado, with Title XIX coverage.​

Additional Information: Private Pay Bookings Available/ Minors transportation.

We will transport at a private pay rate of $3.15 per mile payable prior to your appointment via MC, Amex, Visa. This will be paid in full ahead of time and is nonrefundable in the event of cancellation in less than 48 hours notice.
