Items needed to apply for an EMAP application
Eligible applicants will need the following items:- Identification for all household members 18 years or older residing in the home as their primary residence.
- Evidence of Ownership and Primary Residence (Mortgage Statement)
- Confirmation of Financial Hardship Due to COVID-19
- Proof of Income for all household members 18 years or older residing in the home as their primary residence.
- Evidence of Amount Due (Mortgage Statement or current bill for other assistance types)
- Confirmation that your loan was not a jumbo loan at the time your loan originated (on a case by case basis, documentation of your loan origination amount may be requested)
- Completed Third Party Authorization Form
